Terms of Use Agreement

  1. General

Wellington-Dufferin-Guelph Public Health (WDG Public Health) maintains the Enrollment List for Public Health (“ELPH”) website that enables child care and schools to electronically submit enrollment information.

When you click the “I Accept” button you are agreeing to comply with the following terms of use. Please review the following terms carefully. If you do not agree with these terms you cannot use or gain access to ELPH.

  1. Limited Right to Use

Subject to the terms and conditions of this Agreement, you are granted the right to access and use ELPH for your personal, non-commercial use.

You will only enter information to ELPH that you know to be accurate. You will report any errors in or concerns about ELPH to WDGPH’s Vaccine Preventable Disease Program by mail, fax or in person as set out in section 1 of this agreement.

You are responsible for safeguarding your login credentials and enrollment information, before, during and after your use of ELPH.

You are responsible for the security of the device, browser and network that you are using to access the ELPH website and transmit information.

You will provide accurate ELPH login credentials where required.

  1. No Harmful Use

You warrant that the information or material that you provide to ELPH through your access to or use of ELPH does not infringe the rights of any person or entity.

Without limiting the generality of the foregoing you shall not:

  • Impersonate any other person or entity or make any misrepresentations
  • Unlawfully delete or revise any material posted by any other person or entity
  • Use ELPH in such a manner as to gain unauthorized entry or access to information on ELPH, to post unauthorized information on ELPH or to gain unauthorized entry or access to any password-protected or restricted area of the ELPH or the City’s computer or network systems
  • Interfere with or disrupt ELPH or computer networks connected thereto
  • Violate any law or regulation or any generally accepted Internet practice or interfere with any other legitimate ELPH user’s ability to use ELPH and any of the services or provided by ELPH
  1. Non-Transferability and Passwords

Any password or right given to you to use ELPH is not transferable.

  1. No Liability

WDG Public Health is not liable for any damages whatsoever arising out of your use of ELPH.

  1. Disclaimer

WDG Public Health does not represent and warrant that ELPH will be available or meet your requirements, that access will be uninterrupted, that there will be no delays, failures or errors or omissions or loss of transmitted information. You have sole responsibility for adequate protection and backup of your data, hardware and software and to take reasonable and appropriate precautions against viruses and other contaminating or destructive properties.

  1. Collection, Use and Disclosure of Personal and Personal Health Information

The Municipal Freedom of Information and Protection of Privacy Act (MFIPPA) and the Personal Health information Protection Act (PHIPA) apply to all information collected by WDG Public Health. The information will only be collected, used and disclosed in accordance with MFIPPA and PHIPA. Please refer to the WDG Public Health Privacy Policy for more information.

This information is collected under the authority of the Immunization of School Pupils Act (ISPA) and the Child Care and Early Years Act.

  1. Copyright

The text, images and data on ELPH are protected by the Canadian Copyright Act. WDG Public Health grants you a personal non-exclusive license to print copies of your enrollment information.

You do not gain any ownership rights to any of the copyrighted information you gain access to through ELPH.

  1. Termination of ELPH Use

WDG Public Health may, at its sole discretion, terminate or refuse to permit the use of any services on ELPH by any person, for any reason and without notice.
